Consent to Electronic Communications
By creating an account, providing your email address or phone number, or otherwise engaging with Proveta, you consent to receive communications from us electronically. This Consent is provided under the federal Electronic Signatures in Global and National Commerce Act ("E-SIGN Act") and describes the terms of electronic delivery.
Scope of consent
You agree that we may provide you with any and all notices, disclosures, agreements, receipts, billing statements, privacy notices, clinical communications, and other information (collectively, "Communications") electronically, rather than in paper form. Communications may be delivered by email, SMS, push notification, through the member portal, or by other electronic means.
Types of communications
- Transactional: account creation, order confirmations, shipping updates, receipts, and billing notices.
- Clinical: messages from your provider or care team, prescription updates, and refill reminders.
- Service: security alerts, policy updates, and service changes.
- Marketing: promotions, newsletters, and product announcements (you can opt out of these at any time).
SMS / text messaging terms
By providing your mobile number, you consent to receive recurring automated text messages from Proveta at that number, including marketing messages where you have opted in. Consent is not a condition of purchase.
- Message frequency varies. Message and data rates may apply.
- Reply HELP for help, STOP to cancel.
- You can unsubscribe at any time by replying STOP; we may send one confirmation message.
- Carriers are not liable for delayed or undelivered messages.
- By opting in, you represent that you are the account holder of the mobile number or have the account holder's permission.
Hardware and software requirements
To receive and retain Communications electronically, you need a device with Internet access, a current web browser, a valid email address, and sufficient storage space to save them or printer capability to print them. You are responsible for keeping your contact information current.
Withdrawing consent
You may withdraw your consent to receive electronic Communications by contacting [email protected]. Withdrawing consent may prevent us from providing the Service and may result in termination of your account.
Updating contact information
It is your responsibility to keep your email address and mobile number current. If your contact information changes, update it through your member portal or by contacting us at [email protected].
