SMS Messaging Terms
These SMS Messaging Terms supplement our Terms of Service and apply when you enroll to receive text messages from Proveta.
Program description
When you opt in to our SMS program, you will receive text messages from Proveta about your account, orders, appointments, care, and, if you choose, promotional content. Messages are sent by automated technology. Consent is not a condition of purchase.
How to opt in
You can opt in by providing your mobile number and agreeing to the SMS terms during account creation, checkout, or through your member portal. By opting in, you represent that you are the account holder of the mobile number or have the account holder's permission.
How to opt out
You can opt out of marketing SMS at any time by replying STOP to any marketing text message. After you reply STOP, we will send a confirmation message. Transactional messages (account, order, care) may continue because they are necessary to deliver the Service.

Message and data rates
Message frequency varies. Message and data rates may apply according to your mobile plan. Check with your mobile carrier. We are not responsible for any charges you may incur.
Supported carriers
Carriers are not liable for delayed or undelivered messages. Delivery is subject to transmission limitations and carrier coverage. Supported carriers include AT&T, T-Mobile, Verizon, US Cellular, Boost, Cricket, Metro, and others; carriers may change.
Privacy
Your mobile number and messaging activity are treated according to our Privacy Policy and, where applicable, HIPAA.
